DISCLOSURE
By completing the Master Account Agreement, I am requesting that you
establish a deposit account in my name. If this account is for more
than one individual, the terms “I” and “my” will be interpreted as
“we” and “our” and will apply to all individuals signing the
Agreement. I understand that this account is subject to the Business Account Terms and Conditions. I acknowledge that
I have reviewed these terms and conditions and agree to be bound by
them now and as they may be changed from time to time. I further
understand that the establishment of this account is subject to the
approval of Beach Business Bank, and authorize you to check my
credit, employment history, and inquire into past banking
relationships to obtain the information necessary to provide this
approval.
I certify that the information provided on the Master Account
Agreement is correct to the best of my/our knowledge.
