DISCLOSURE
By completing the Master Account Agreement, I am requesting that you
establish a deposit account in the name of a Trust. If there is more
than one Trustee, the term “I” and “my” will be interpreted as “we”
and “our” and will apply to all Trustees signing the Agreement. I
understand that this account is subject to the
Personal Deposit Account Terms and Conditions. I acknowledge that I have
reviewed these terms and conditions and agree to be bound by them
now and as they may be changed from time to time. I further
understand that the establishment of this account is subject to the
approval of Beach Business Bank, and authorize you to check my
credit, employment history, and inquire into past banking
relationships to obtain the information necessary to provide this
approval.
I certify that the information provided on the Master Account
Agreement is correct to the best of my/our knowledge.
